When people hear "herbal medicine" many imagine a simple cup of herbal tea. In Unani medicine, the reality is far richer. Unani herbal medicine is a complete, organized system of pharmacology, with its own principles for identifying, testing, purifying, combining, and prescribing natural substances. It has been refined over more than a thousand years, and it is the foundation of my daily practice.
Let me explain what it truly is in the classical tradition, in the Prophetic guidance, and in the light of modern science.
A SCIENCE OF NATURAL SUBSTANCES:
Unani medicine recognizes three sources of medicine, Plant (Nabati), Mineral (Madani), and Animal (Haywani). The vast majority are plants, Based, roots, seeds, flowers, barks, fruits, and resins, but the system also includes purified minerals and animal products, each with strict rules for preparation.
Every substance in the Unani pharmacy is studied through its temperament (Mizaj). Just as every person has a temperament, Hot, Cold, Dry, or Moist in different degrees, so does every herb. A "hot and dry" herb in the third degree, for example, acts very differently from a "cold and moist" herb in the first degree. This is why, in my practice, two patients with the same complaint may receive different herbs: the medicine must match the patient's temperament, not only the symptom.
This temperament-based prescribing is what separates Unani herbal medicine from the casual use of herbs. It is precise, individualized, and deeply logical.
THE ONE DESIGN OF ALLAH (God): FOUR NATURES, FOUR SEASONS:
SubhanAllah, glory be to Allah (God). Look at the perfection of His design. The world is built upon four natures: heat, cold, dryness, and moisture, each in its degrees. Fire is hot and dry, air is hot and moist, water is cold and moist, earth is cold and dry. And the human body? Allah created it upon the very same four temperaments: blood (Dam) hot and moist like air, yellow bile (Safra) hot and dry like fire, phlegm (Balgham) cold and moist like water, black bile (Sauda) cold and dry like earth. The world and the body speak one language — so that the world itself becomes a support for the body. Alhamdulillah, all praise belongs to Allah (God), who placed the remedy in the same design as the disease.
See also the four seasons in their perfect rotation: when summer's heat becomes severe, Allah (God) brings autumn; when autumn's dryness grows severe, He brings winter; when winter's cold becomes severe, He brings spring — and no season is ever allowed to rule forever. The same science repeats inside us: severe heat must be cooled, severe cold must be warmed, dryness calls for moisture. And for this, Allah gave us two precious gifts — the herbs of the earth, each carrying its own temperament, and the mind to understand and use them. The physician who matches a herb's nature to the patient's nature is only reuniting what Allah created as one system. This is not man's invention; it is man's intellect reading the book Allah already wrote. For all of this, we thank Him.
And reflect on the weathers of the year — each season carries its own temperament, and each one stirs the humours within us. Spring arrives hot and moist, and the blood rises; this is the season of balance and renewal. Summer comes hot and dry, stirring the yellow bile — thirst grows and tempers shorten. Then autumn turns cold and dry, and the black bile gathers; this is the season when heaviness and worry visit many hearts. Winter settles cold and moist, and phlegm increases — the body slows and seeks warmth and rest. The wise physician watches the sky as he watches the pulse, for the weather outside is always speaking to the temperament inside. And for every season's excess, Allah (God) placed a herb of the opposite nature in the earth. The system is complete — nothing is missing.
SINGLE DRUGS AND COMPOUND FORMULATIONS
Unani pharmacy distinguishes between two categories:
Mufrad (single drugs) — one herb used alone, such as black seed, senna leaves, or rose petals. The classical texts describe each single drug's temperament, actions, and correct dose in detail.
Murakkab (compound formulations) — carefully designed combinations like Majun, Jawarish, Khamira, and Itrifal. These are not random mixtures. Each ingredient has a role: the main active herb, supporting herbs that strengthen its action, and corrective herbs that soften any harshness. A good Murakkab is like a well-trained team — every member has a job.
Many of the classical formulations I dispense — from Hamdard, Qarshi, and other trusted makers — follow recipes recorded centuries ago by physicians like Ibn Sina and Al-Razi.
PURIFICATION BEFORE PRESCRIPTION
One of the most sophisticated ideas in Unani pharmacy is Tadbir — the purification and processing of drugs before use. Some potent substances are detoxified through specific traditional processes; others are "potentized" by grinding, roasting, or combining them with correctives.
The principle is honest and humble: nature provides powerful tools, but the physician must prepare them wisely. A raw herb and a properly prepared Unani medicine are not the same thing. Preparation is part of the medicine.
HERBS IN THE PROPHETIC TRADITION
For Muslim patients, Unani herbal medicine carries the fragrance of the Sunnah. The Prophet Muhammad (peace be upon him) guided his Ummah toward several natural remedies that remain central to Unani practice today.
The most famous is the black seed (Nigella sativa, known as Kalonji or Habbatus Sauda). The Prophet (PBUH) said, in a hadith recorded in Sahih al-Bukhari and Sahih Muslim: "In the black seed there is healing for every disease, except death." Modern researchers have studied its active compound, thymoquinone, with great interest — a beautiful meeting of Prophetic guidance and laboratory science.
Honey is another: the Quran itself describes honey as "a healing for people" (Surah An-Nahl, 69), and the Prophet (PBUH) recommended it. Senna leaves (Sana Makki) were also mentioned in his guidance for cleansing. These are not folk tales — they are documented traditions that Unani physicians have studied, prescribed, and refined for centuries.
WHAT MODERN SCIENCE SAYS
Modern phytochemistry — the study of plant chemicals — has validated what Unani physicians long observed. We now know that herbs contain active compounds: alkaloids, flavonoids, tannins, essential oils, and glycosides. When a traditional text says a herb "strengthens the heart," the laboratory may find cardiotonic glycosides. When it says a herb "calms the nerves," researchers may find sedative flavonoids.
Standardization, quality testing, and clinical observation are bringing the two worlds closer every year. In my view, science does not replace the Unani tradition — it translates it.
NATURAL DOES NOT MEAN CARELESS
One misunderstanding I correct almost daily: "natural" does not mean "harmless." A potent herb in the wrong temperament, wrong dose, or wrong combination can cause harm. This is why self-prescribing from the internet is risky, and why the classical system insists on assessment first — Nabz (pulse), Mizaj (temperament), and a proper history.
A herb is a medicine. Treat it with the respect a medicine deserves, and it will serve you well.

After reading the pulse and assessing temperament, I select single herbs or classical formulations matched to the individual — considering their constitution, lifestyle, diet, and condition. I source from makers I trust, and I explain to every patient what they are taking and why. No mystery, no guesswork.
Unani herbal medicine is ancient, but it is not outdated. It is a living science — rooted in the wisdom of a thousand years, guided by the Sunnah, and increasingly confirmed by the laboratory. That is what I practice, and that is what I offer my patients.
